ForthOS — Universal Binary Translator pipeline

Five-stage pipeline: Windows .sys / PE binary → PE loader → x86 decoder → UIR lifter → Forth codegen → named Forth vocabulary blocks. End-to-end proven (commit 243b0f0): 145/145 translator tests + 8/8 e2e tests.
